We
offer two convenient options when refilling your prescription:
Please
be aware that regular medical supervision is required to
refill prescriptions. You are required to schedule a follow-up
appointment for you (or your child) prior to requesting a
prescription refill.
Please Note: Controlled substances cannot be called
in. They must either be mailed or picked up in our office.
These prescriptions must be filled within one month and no
refills are allowed.
Please call our office for your prescription refill during
normal business hours, Monday - Friday between 9:00 am and
5:00 pm and leave your message on our prescription voicemail
line. If phone requests are obtained after 2:00 pm, your
prescription will be handled the next business day. Please
talk slowly and clearly and provide the following information:
- Patient's first and last name, with proper spelling.
- Medication(s) being requested.
- Phone number where you can be reached, in the event of a
problem.
- Pharmacy name and phone number.
- Indicate if you want us to call in the prescription,
will pick it up or would like it mailed. Please note that there is a $2.50 fee for mailing your prescription.
By agreeing to the terms of our OnLine
Rx
form, you will enter a secure area of our site. Suburban Psychiatric Associates,
LLP. will be held harmless should your request not
get transmitted due to technical problems related to the
Suburban Psychiatric Associates, LLP. site. All
personal identifying information is encrypted and your message
will not be internally or externally forwarded to other third
parties. The information will be used only by Suburban Psychiatric Associates,
LLP.
We
will respond to your request within 48 hours, Monday - Friday,
during business hours, 9:00 am - 2:00 pm. For those
prescriptions received after 2:00 pm, your prescription will
be handled the following business day. Prescription refills
are NOT done on weekends and holidays. For those prescription
requests received on Friday and weekend days, your
prescription will be handled on the next business day. If
you do not hear from us within that timeframe or should have
any questions regarding your prescription refill or this
on-line form, please contact us by phone at (716) 689-3333.
If
you do not accept the terms of this site disclaimer, you will
not be able to process your prescription request on-line.
